Q&As on Applying for Health Code (July 2022)

1. How to request Health Code?

1.1 Chinese nationals (including the residents of Hong Kong SAR, Macao SAR and Taiwan) can apply for the Health Code through the Wechat mini-program, which can be accessed by scanning the QR code below.

1.2 New Zealand citizens and other foreign nationals can log on the following website to apply for the Health Code https://hrhk.cs.mfa.gov.cn/H5/


2. What documents are required for health code application?

2.1 Required documents prior to travel

Please provide two negative PCR test results. Please refer to section 3 and 4 for the time frame of the tests and other requirements. (Testing facilities can be found and book through the link https://testtotravel.co.nz )

2.2 Vaccination Certificate. Not required to unvaccinated or not fully vaccinated people.

    

2.3 Proof of residence and Visa

1) Chinese nationals whose ordinary place of residence is New Zealand (including the residents of Hong Kong SAR, Macau SAR and Taiwan) should upload their New Zealand visas or residence permits (Please refer to Section 6). 

2) New Zealand citizens whose ordinary place of residence is New Zealand, please upload the information pages of your valid passports and valid Chinese visas issued after 28 March 2020, or valid Chinese residence permit (employment, private matters and family reunion).

3) National of a third country whose ordinary place of residence is New Zealand, please upload the information pages of your valid passports, and valid Chinese visas issued after 28 March 2020, or valid Chinese residence permit (employment, private matters and family reunion), and valid New Zealand visa or residence permit.

2.4 Travel itinerary

Electronic ticket or flight itinerary issued by airline company or travel agent bearing full name of the passenger and detailed flight information. The ticket must show the origin of departure is from New Zealand to China. Flight itinerary without personal information and flight details, and screenshot of flight booking from text message, are not acceptable.


3. How to choose the date for the tests?

1) Two separate COVID-19 PCR tests are required within two days prior to boarding the China-bound flight. The two COVID-19 PCR tests must be conducted by different testing facilities.  The sampling for the second test must be administered after 3pm on the day before boarding your scheduled flight to China.  If the two tests have been conducted by the same testing facility, the two tests must be administered with a sampling interval of over 24 hours.  

2)  When requesting the health code, please use the sampling time of the first PCR test as the time of testing.

Please be aware that there are possibilities that your test reports cannot be completed on time as some testing facilities do not open during the weekends. Please make your travel arrangement in advance and consult your local testing facilities.


4. What are the requirements on the test reports?

The Chinese diplomatic and consular missions in New Zealand only accept a negative PCR test report conducted with a nose or throat swab specimen (Nasopharyngeal Swab). 

The original COVID-19 PCR test reports issued by a testing facility are required when requesting for the health code. The reports must clearly show your full name (must match the full name shown on your passport or travel document), date of birth, and date of tests (sampling), and test results, and name of the testing facility. If you are unable to provide the original COVID-19 PCR test reports, reports issued by a GP or a clinic (including Inbox reports) are acceptable but must distinctly state the name of the testing facility, and be signed by the GP or carry the stamp of the clinic.

Reports will not be accepted if any required information is missing or without the above-mentioned signature or stamp. Test results send by text message are not acceptable.


5. What are the acceptable travel documents?

When applying for the health code, New Zealand citizens or citizens from a third country, should submit a copy of valid passport. Mainland Chinese citizen should submit a copy of valid Chinese passport. Residents of Hong Kong SAR, Macao SAR and Taiwan should submit the corresponding Mainland Travel Permit for Hong Kong and Macao Residents, or Mainland Travel Permit for Taiwan Residents, or Chinese Travel Document.

Please make sure that the number of travel document you provided matches the number shown on your test report, vaccination certificate, proof of residence, and travel itinerary.

6. What if I do not have a valid proof of NZ residence?

If your New Zealand visa or residence permit has expired, please provide a copy of the expired visa and complete the declaration of overstaying in New Zealand (refer to Appendix 1).


7. When should the application for health code be uploaded?

Please be advised to submit the application through a proper platform upon receiving the test reports and pay close attention to the outcome of the application. If the application has been declined (shown in red color), please provide information as shown on the notification and make the request again.

When you have all required documents ready and please apply for the health code as soon as possible but no later than 7pm on the same day of your scheduled flight. Please make sure you have been issued a health code before arriving at the airport and to avoid missing your flight.

All applications for the health code will be reviewed manually, and may take a longer time to process. Please check your status of your request through the app or website, and only contact us in case of emergency.


8. Do you have any tips for submission of personal information?

8.1 Applicants who are the Chinese citizens (including the residents of Hong Kong SAR, Macao SAR and Taiwan) should enter their full names in Chinese as shown on travel documents as well as the travel document numbers. The residents of Hong Kong SAR, Macao SAR and Taiwan should also upload a picture of the travel document.

8.2 Please provide your New Zealand local mobile phone number, or if you don’t have a local phone number, you have provided your Chinese mobile phone number, please make sure the number you provided is reachable, in case we need to talk to you.

8.3 Please submit the application for the health code to the mission whose consular service covers the location of the testing facility, as shown below:

Chinese Embassy in New Zealand: Wellington, Manawatu-Wanganui, Hawke's Bay, Taranaki, Gisborne, Bay of Plenty

Chinese Consulate-General in Auckland: Auckland, Northland, Waikato

Chinese Consulate-General in Christchurch: South Island

9. What could he/she do if he/she made a mistake in submitting the materials for the health code, or the application were declined?

He/she will be notified of the reasons for any failure of application through the Wechat mini program or H5 website. He/she is requested to re-submit the application for the health code after making due adjustments as required.


10. Does he/she need to bring the hard copies of test reports when boarding flight to China after receiving the green health codes?

He/she is recommended to bring the hard copies of the negative test reports, vaccination certificates, Personal Health Monitoring Form, and Declaration of Commitment on COVID-19 Close-Loop Management and Personal Health Monitoring in case of inspection. He/she may be declined to board if he/she refused to cooperate with the airline when verifying documents.


11. Do infants have to apply for the health code?

Children under the age of 5 years old (on the day of departure) are required to apply for the health code by providing one negative PCR test report which the sampling has been taken after 3pm on the day before boarding flights to China, along with other required documents, provided that an accompanying legal guardian has received a valid health code for the China-bound flight from New Zealand. (Other documents are referred to 2.3 and 2.4).


12. How does a seafarer apply for health code?

Seafarers who disembark in New Zealand and depart for China from Auckland do not require to enter a 14-days self-isolation, and seafarers can request the health code in accordance with current requirements of pre-departure COVID-19 tests. 


13. People who have been infected with COVID-19.

If you have been diagnosed with COVID-19, or have not been confirmed case of COVID-19 but received a positive PCR test result, you are considered   infected with COVID-19. The procedure are as follows:

13.1 Recovery Pre-check

Please send the confirmation of diagnosis or positive COVID-19 PCR test report, and two negative COVID-19 PCR test reports (samples must be taken at 24 hours intervals), and a copy of the information page of the valid passport for the pre-check purpose to the Chinese Embassy or Consulate-General in accordance with consular district jurisdiction.

Please put “Full name + Passport Number + Nationality + Recovery Pre-check” into the subject line when sending an email, and the content of your email should also include your local contact number and the information of your intended flight. You are responsible for not getting a response from us in case of sending your email to a wrong office or are not in compliance with the requirement of the email subject.

13.2 Health Monitoring

You must go into self-isolation and monitor health for 14 days, starting from the next day once the  recovery pre-check process has been confirmed, and also complete the Personal Health Monitoring Form (refer to Appendix 2). After completing the self-isolation and health monitoring, you can then request for the health code as regular passenger, and you do not need to confirm with us again, however, please make sure you have uploaded all required documents, including the documents showing you have fully recovered.

Please contact the Chinese Embassy or Consulate-General in New Zealand in advance to avoid travel delays.


14. Close contact or people showing suspect symptom

You are considered as a close contact if you contacted someone who is a suspect case or a confirmed case 2 days before the person showing symptoms, or contacted someone who is asymptomatic 2 days before the sampling has been taken. For example, a person who is an accompanying family member, or a colleague, or a classmate, or a person who lives together, of a confirmed case or asymptomatic (confirmed by a positive COVID-19 PCR test result), then the person is considered as a close contact.

In the past of 14 days, if you had any suspect symptoms, such as fever, cough, diarrhea, or had been declined to board a flight as a result of high body temperature or suspect symptoms, you are considered as a suspect case.

Close contact and suspect case must enter a 5 days self-health monitoring, starting from the next day after being identified as a closed contact or has shown the suspect symptoms,  and complete the Personal Health Monitoring Form (refer to Appendix 3).  You can request the health code by providing two PCR test results as the required timeframes only if you did not have any symptoms during the period, and make sure to upload your diagnosis, Personal Health Monitoring Form, and all test reports when requesting the health code.


15. Flying to China via a third country?

Passengers can transit through New Zealand to China, and transiting passengers are required to request the health code at the origin country of departure and the country of transiting.

Auckland International Airport does not have any PCR testing facilities, people from a third country intending to travel to China via New Zealand, must enter New Zealand, and New Zealand will be considered as the origin country of departure, and then request the health code by meeting the two PCR tests requirement set by the Chinese diplomatic and consular missions in New Zealand. Please be advised, that in order to avoid being stranded, you may need to arrange a proper New Zealand visa to enter New Zealand. You must also have received a health code from the origin country of departure before requesting the health code through the Chinese diplomatic and consular missions in New Zealand.

Passengers intending to transit through a third country to China from New Zealand, you must request the health code through the Chinese diplomatic and consular missions in New Zealand. You must also request the health code in compliance with the requirements set by the Chinese diplomatic and consular missions in the transiting country during transit, please contact the Chinese diplomatic and consular missions in the transiting country prior to your travel, and make sure you plan ahead and avoid being stranded
